These Terms and Conditions (“Terms”) govern your access to and use of the Align PT & Pilates website, services, facilities, and any related communications, content, or offerings provided by Align PT & Pilates (“Align,” “we,” “us,” or “our”). By accessing our website, scheduling an appointment, participating in services, or otherwise engaging with Align PT & Pilates, you agree to be bound by these Terms.

If you do not agree with any part of these Terms, please do not use our website or services.

1. About Align PT & Pilates

Align PT & Pilates is a boutique physical therapy and Pilates practice located at:

Our services include, but are not limited to:

• Physical Therapy

• Orthopedic and Sports Rehabilitation

• Rehab Pilates and Fitness Pilates

• Women’s Pelvic Floor Therapy

• Manual therapy, dry needling, and related modalities

• Group and private Pilates instruction

• Virtual consultations (when offered)

All clinical services are provided by licensed healthcare professionals or qualified instructors under appropriate supervision.

2. Medical Disclaimer

2.1 Not Emergency Care

Align PT & Pilates does not provide emergency medical services. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately.

2.2 No Guarantee of Results

While our services are grounded in evidence-based practice and clinical expertise, individual outcomes may vary. We do not guarantee specific results from physical therapy, Pilates, pelvic floor therapy, or any related services.

2.3 Educational Content

Any information provided on our website, social media, emails, or other platforms is for general educational purposes only and is not intended to replace professional medical advice, diagnosis, or treatment.

3. Client Responsibility & Informed Participation

By participating in services at Align PT & Pilates, you acknowledge and agree that:

• You are responsible for providing accurate and complete health information.

• You will disclose all relevant medical conditions, injuries, surgeries, medications, and pregnancy status.

• You will notify us immediately of any pain, discomfort, dizziness, or adverse symptoms during or after sessions.

• You understand that physical activity and therapeutic interventions carry inherent risks.

Participation in services is voluntary and undertaken at your own discretion.

4. Assumption of Risk

Physical therapy, Pilates, exercise, and manual therapy involve physical movement and exertion that may include inherent risks such as soreness, strain, fatigue, aggravation of symptoms, or injury.

By engaging in our services, you:

• Acknowledge these risks

• Assume full responsibility for your participation

• Release Align PT & Pilates and its staff from liability for injuries resulting from failure to follow instructions or disclose relevant health information

5. Appointments, Scheduling & Attendance

5.1 Appointment Scheduling

Appointments may be scheduled online, by phone, text, email, or in person. Availability is limited due to our one-on-one and small-group model.

5.2 Late Arrivals

Late arrival may result in a shortened session to avoid impacting subsequent appointments. Full session fees may still apply.

5.3 Cancellations & No-Shows

Clients are expected to provide advance notice for cancellations or rescheduling.

Failure to do so may result in:

• A cancellation fee

• Loss of prepaid session

• Full charge for the scheduled service

Specific cancellation policies may vary by service type and are communicated at scheduling.

6. Payment, Fees & Billing

6.1 Payment Responsibility

Clients are responsible for payment of all services rendered, including any portion not covered by insurance.

6.2 Insurance

• We accept Medicare for Physical Therapy and Rehab Pilates when medically appropriate.

• We are in-network with United Healthcare.

• We bill out-of-network insurance when benefits are available.

• A superbill may be provided for clients without out-of-network benefits.

Insurance coverage is not guaranteed. Verification of benefits does not guarantee payment.

6.3 Self-Pay Services

Self-pay rates are available and due at the time of service unless otherwise arranged.

7. Packages, Classes & Prepaid Services

• Class packages and prepaid services are subject to expiration terms disclosed at purchase.

• Packages are generally non-refundable and non-transferable unless otherwise stated.

• Missed sessions may result in forfeiture of that session.

8. Minors Policy

Align PT & Pilates treats youth athletes ages 10 and up.

For minors:

• A parent or legal guardian must be present during sessions unless otherwise approved.

• Consent forms must be completed prior to treatment.

• The guardian assumes responsibility for the minor’s participation.

9. Pelvic Floor & Sensitive Care Consent

Pelvic floor therapy may involve internal or external assessment and treatment.

By scheduling pelvic floor services, clients acknowledge:

• Informed consent will be obtained prior to treatment

• Treatment will be explained clearly

• Consent may be withdrawn at any time

Client dignity, privacy, and comfort are always prioritized.

10. Virtual Consultations & Telehealth (If Applicable)

Virtual consultations:

• Are informational and not a substitute for in-person care

• May be limited in scope due to a lack of physical examination

• Require reliable internet access and privacy on the client’s end

11. Intellectual Property

All website content, including text, images, videos, logos, branding, and educational materials, is the property of Align PT & Pilates unless otherwise stated.

You may not:

• Copy, reproduce, distribute, or modify content without written permission

• Use our content for commercial purposes without authorization

12. Website Use & Acceptable Conduct

You agree not to:

• Use the website for unlawful purposes

• Submit false or misleading information

• Interfere with website functionality or security

• Attempt unauthorized access to systems or data

We reserve the right to restrict access for violations.

13. Privacy & Confidentiality
